Mortally wounded aboard "BASKETBALL 813", Marine Corps KC-130F BuNo QD 149813 while serving with VMGR-152, MAG-15, 9th MAB, MCAS Futenma, Okinawa, Japan.

Jerry was transported to Texas for critical care, but died later of complications related to his wounds.

He is listed on the Vietnam Veteran’s Memorial in Washington DC, Panel 42E/15

Jerry is buried at Jefferson Barracks National Cemetery, St. Louis, Missouri.
